What is Home Health Care?

Home Health Care is skilled nursing care, therapy, and aide services provided in the comfort and privacy of your home for the treatment of an illness or injury. Care is delivered by RNs, LPNs, Physical Therapists, Occupational Therapists, Speech Therapists and aides, as needed. Medicare, Medicaid, Veterans Administration and Private Insurance pays for needed services.

What is Personal Care?

Personal Care includes bathing, laundry, light meals preparation, light cleaning, assist client with short distance transfer and ambulation if needed. Care provided in the comfort and privacy of your home. Without any coverages, Private Pay is the usual means of payment for services.

What is Residential Care?

Residential Care is the personal care given to persons with developmental disabilities, to assist the client meet his or her daily living needs in the client's residence and within the community. Medicaid eligibility is required for payment of Habilitation Training and Daily Living Supports

What is Adult Day Care?

Day Care for the elderly, also called Adult Daily Activities Center, offering respite to family caregivers. Our participants and their entire families benefit from Caregiver Solutions Daily Activities Center. Open from 7 a.m. to 6 p.m. Monday through Friday. The center offers health monitoring, with an R.N. and L.P.N. on staff, medication administration if needed, personal care with hygiene, such as shampoo, haircuts and nail care, light exercise and therapeutic care. Participants also enjoy music and some TV programs, art and craft, games, entertainment and a good meal. Plus, they benefit from socialization with others, varieties of activities, leading to improved mental and physical health and, for many, an overall improvement in the quality of life.

Transportation is provided to and from our center for most clients. The family caregivers are free to go to their jobs or otherwise get on with life, secure in the knowledge that their loved ones are cared for and happy during the day.

Am I eligible for Home Health Care?

There are four conditions that must be met in order for you to be eligible for home health care services: Remember it is the patient's choice when selecting home health provider for services.

How do I receive home health care?

Express your interest by discussing Comfort Living Home Health with your discharge planner, social worker, or physician. Before care begins you will receive a free in home assessment that will help you identify your specific needs, discuss your care plan, and resources available in your community. An individualized plan will be developed and approved by your physician outlining the care to be provided.

Contact us, Comfort Living Home Health at (405) 735-6122 to help you assess your need for home health care. Then, with your physician's orders, your services begin as ordered.

How quickly can services be provided?

Most patients are seen within 24 hours of referral. After a complete assessment of your health care needs, your doctor instructs us on the level of skilled nursing services that will meet your needs. We don't stop until your needs are met and you or your doctor ends your care.

Where are services provided?

Care is provide in the home, as well as assisted living facilities and retirement communities. The patient's privacy in a comfortable home setting promotes more efficient delivery of care.

When are services available?

Care is available seven days a week. We also have Registered Nurse's on call 24 hours a day.

How long do services last?

Services may last a few weeks to several months. Care ends and patients are discharged from our services when goals are met or when services are no longer needed.

Can I change the scheduled visit if I have an appointment or other engagements?

Changes can easily be made by calling our office. Visits can be rescheduled for a time that is convenient for you. Your care plan will be both flexible and conveniently designed to ensure best results in you care.

Will my insurance cover Comfort Living Home Health services?

Our exceptional home health care services are well within reach for most patients. Medicare, private insurance, Managed Care Plans, Veterans Administration, and Workers' Compensation typically cover many of our services.

Do you offer help with filing insurance claims?

Yes. Our reimbursement specialists conveniently provide assistance with insurance verification, authorization, reimbursement levels and other insurance issues.

If I have a concern that needs to be addressed with someone other then my caregiver, whom do I contact?

Please contact Comfort Living Home Health at (405) 735-6122 and your call will be referred to the appropriate party. Either our Director of Nursing or agency's Administrator will listen to your concerns and they will work to resolve any issues to the best of their abilities.
